Use this checklist to evaluate whether it is safe to proceed with a home visit. This form should be completed before the visit via a conversation with the participant over the phone or via video conferencing.
Do not enter the property if you believe that this would put yourself or anyone else at risk. Contact your manager if you are attending an appointment but think entering a participant’s home would be unsafe.
